How to refer a patient to our center
To refer a patient to the ALS Center at UCSF who resides in the United States, you have two options:
- Use MD Link, our secure portal for physicians.
- Download our referral form, complete it and fax it to the clinic at (415)-353-9060.
Be sure to fax (415) 353-2524) your medical records to us before the date of your patient's visit. If you need assistance, please give us a call at (415) 353-2122
Health Insurance
Be aware that many insurance plans will require an official insurance referral from a primary care physician before a patient can schedule a visit with an outside specialist at UCSF. Contact your patient's insurance to learn whether you need to obtain an authorization prior to their visit.r
